Clinical Director

About Onos Health

Onos Health’s mission is simple but ambitious: ensure every healthcare dollar goes toward delivering the highest quality care. Today, nearly 30% of U.S. healthcare spending is wasted due to ineffective care and administrative burden caused by misalignment between providers and payers.

Onos is addressing this by building the AI platform that enables health plans to understand clinical care at scale. By transforming unstructured clinical documentation into actionable insights, we help payers make faster, more informed decisions, identify opportunities to improve quality and reduce waste, and ensure more healthcare dollars are directed toward delivering better outcomes.

Come join a category-defining company and transform how healthcare is managed in the U.S.

Why Onos

  • Meaningful impact: Help solve one of healthcare’s biggest challenges by improving quality while reducing waste.

  • Ownership: Take on significant responsibility from day one and shape a rapidly growing company.

  • Direct collaboration: Work alongside experienced founders with deep expertise in healthcare, AI, and product development.

  • High-performing team: Join a transparent, collaborative, and execution-focused culture.

  • Build something new: Help define the next generation of AI-powered healthcare analytics.

The role

Onos builds healthcare AI for health plans, and our next build is in the hardest part of behavioral health: residential treatment and inpatient care. We’re looking for a Clinical Director who has worked in those settings to help us build the AI models behind it.

You’ll work side by side with our data science and product teams. You decide what good clinical judgment looks like for higher levels of care, make sure the models reflect it, and tell us when they don’t. You’ll start as an individual contributor and work alongside the clinical leads for ABA and lower levels of care.

What you’ll own

  • Clinical voice on the AI product: tell product and engineering what’s clinically valid, what isn’t, and why, for patients in higher-acuity settings

  • Guardrails and escalation: define when the AI acts, when it flags, and when it hands off to a clinician

  • Clinical review: evaluate AI outputs against real clinical standards and help build the review process

  • Payer and partner conversations: explain the clinical model to health plans and partners and answer hard questions about safety and outcomes

  • Help operationalize clinical guidelines and practices that may exist inside individual clinical judgment and experience at payer customers

  • Standards for higher levels of care: set the clinical approach for RTC and inpatient work, and shape the team that follows you

  • Cross-team alignment: work with the other clinical leads so our approach holds together across all levels of care, from outpatient to ABA to inpatient.

What you bring

  • MD or DO, board-certified or board-eligible in psychiatry

  • Hands-on experience in higher levels of care: residential, inpatient, partial hospitalization, or intensive outpatient

  • Comfort making clinical calls with incomplete information and explaining them to non-clinicians

  • Clear, direct communication with engineers, executives, and payers

Strong plus

  • Startup or digital health experience

  • Experience with Substance Abuse Disorders and Addiction Medicine

  • Health plan or payer experience

  • Experience evaluating or deploying clinical technology

Benefits & Perks

  • Hybrid 3x days per week in office (Mon/Tues/Thurs in San Francisco, Financial District)

  • Flexible vacation policy

  • Paid parental leave

  • Medical, dental, and vision insurance

  • Pre-tax commuter benefits

  • 401(k)

  • Significant equity as an early employee

  • Direct mentorship from experienced founders

  • Ground-floor opportunity to help build both the product and company

  • Regular team events and off-sites

  • Company provided equipment and home office setup

  • In-office lunches and snacks provided
